Follow the evidence shown in your CareCard requirement. Ask a health care provider to review an incomplete or uncertain vaccination history.
What your assigned requirement may ask for
- An official immunization record showing completion of the polio primary series
- The date of each dose used as evidence
- A booster record only when the assigned requirement calls for one
Combination vaccines may include polio. Upload the complete record so the vaccine names and dates can be reviewed.
Current guidance
The Canadian Immunization Guide recommends a three-dose primary series for previously unimmunized health care workers. A single lifetime booster is recommended for health care workers at the highest risk of exposure, such as those with close contact with people who may be excreting poliovirus or laboratory workers handling specimens that may contain it. A booster is not a routine requirement for every health care worker.
